This is a new achievement added in 4.3. Even though it is currently part of the meta for Lunar Festival, anyone who has previously completed To Honor One's Elders will retain credit. Anyone who has not yet completed the meta will have to complete this achievement. Players were similarly grandfathered for Loremaster and Explorer when Cataclysm zones were added at Cataclysm's launch.

I'm surprised this achievement was added to the meta, so far Blizzard has refrained from adding additional achievements to the meta for What A Long Strange Trip It's Been.

According to the achievement in Wow's GUI, the general locations of these Elders are:

Elder Moonlance in Vashj'ir
Elder Windsong in Hyjal
Elder Evershade in Nordrassil
Elder Stonebrand in the Temple of Earth
Elder Deepforge in Deepholm

All the Elders stand inside a pillar of yellow light so they're quite easy to spot.
I only did this achievement this year, because I already have the other ones from the past Lunar Festivals. It took me less than 20 minutes to visit all the Elders (wowhead research included).

I took portals to all the Cataclysm locations. Since there are no portals from Vashj'ir and Uldum to the capital cities, I started in Vashj'ir, hearthstoned back, then I went to Hyjal, Twilight Highlands and Deepholm and just before I was done with the last Elder in Uldum, my hearthstone was ready again.

Too bad I didn't have a mage friend to bring along, this would have taken less time with his portals...

As of post the Lucky Red Envelope are handed straight to you after you honor the elders and are no longer received via mail.

There is also a new achievement called Elders of Cataclysm in which you have to honor the new elders that arrived in the Cataclysm expansion.
In the new Cataclysm locations:

Elder Firebeard is going to be extremely difficult if you're not 80+ due to massive amounts of highly aggressive mobs, including dragons flying over the area that will shoot you down.

I just corpse-crawled it on my 67 Death Knight, but the reason I'm posting is because this handy macro will make the ordeal slightly more bearable (also useful for doing opposite faction Elders)

Your trip (this route works best with all of the zone portals already open)

Head into Vashjir first and head to central Shimmering Expanse for Elder Moonlance, then hearth to avoid the aggravating water world.

Deepholm, Hyjal and Twilight Highlands are interchangeable in their order since they have return portals. If you have a fast hearthstone or mage portals to return to Stormwind or Orgrimmar that's even better.

In Deepholm, talk to Stonebrand the Elder right at the doorway to the Temple of Earth, then fly up to Stonehearth and speak to Deepforge the Elder.

Elder Evershade is just outside the Nordrassil Inn (next to the portal to Stormwind in fact) so don't run far. Then fly on out to the Sanctuary of Malorne.

Twilight Highlands is a straight line for Horde, or a triangular circle for Alliance, either way head just south of Thundermar for Elder Darkfeather and then south to the Dunwald Ruins for Firebeard.
Both of these Elders are used to the conflicts of TwiHi and are surrounded by aggressive mobs so be careful if you don't want to fight.

The Uldum elders are at opposite sides of the zone if you came by portal so it's either or for order.
Elder Menkhaf is in a stairwell on the north side of Khartut's Tomb and Elder Sekhemi is in the northwest corner of the Ruins of Ammon and once done here you can hearth again.

If you have a lower level (not able to fly yet) character that cannot easily access the Cataclysm zones, it is possible to get at least five of the elders in those zones. Through some patience and corpse jumping or having a high level friend escort you.

Uldum is not locked off. You can walk or ride right in through the entrance just a little NW of the Dawnrise Expedition in southern Tanaris.

To get the Twilight Highlands elders, swim or waterwalk along the coast by Fuselight by the Sea in northeastern Badlands. Continue north until you find the shore of the Highlands. Phasing will not matter the elders are still there.

Lastly, it's possible to get the elder in Vashj'ir. Bring some water walking and water breathing potions with you. From Menethil Harbor, go south and follow the northern coast of Dun Morogh to the very west tip. From there it's a straight waterwalk on your mount to Kelp'thar Forest. Waterwalk the rest of the way south to Biel'aran Ridge to where the elder is to avoid the enemies along the way. Drink your water breathing potion and dive to him. There are a few mobs down there near him so you may have to be quick.
