Easily the hardest heroic mode in the entire meta, Nefarian's extra heroic mode abilities really screw with everyone. He has an addition ability in each of the 3 phases. This strategy only applies to 10 man.

Phase 1
In phase 1 and 3, an ability is cast on a random 2 people (not tanks) called Dominion. You cannot move yourself, but are slowly walking to a portal like object. There is a purple beam between you and this portal. This is a mind control-like ability gives you a new action bar with 2 abilities. The first ability breaks it immediately. So what's the point of it then? Well the second ability is a stacking damage buff (a really big one). All DPS want to spam it to stack their buff until they're just about to reach the portal. Then break the ability. Healers want to break it immediately. Players afflicted with dominion become hostile and therefore don't take any damage from Nefarian's electrocutes. These players can also be slowed to prolong the amount of time to spam the buff. If you hit the portal it's an instant death.
Other than that, you want to assign an add tank, not kiter in phase one. The adds move very quickly and will easily rip apart any hunter or mage class that is attempting to kite them. I recommend a warrior or DK. Holy paladins also work well. Have the adds die in one place so that they are easily picked up in phase 3.
Our group DPSed Onyxia down to 2 million and then switched to Nef and brought him to 40 million (2 electrocutes and almost a third). DoTs brought Ony down to about 600k and Nef down to about 38 million. Finish off Onyxia and then get to pillars. If you get an electrocute at the beginning of phase 2 it's going to be an instant wipe. Trust me.

Phase 2
The damage of the lava and the AoE that Nef throws out is significantly higher now. In addition, all 3 adds must die at the same time, because if one dies phase 2 ends, instead of the last one dying. The cast time is now about 3 times shorter, which means very quick interrupts are needed.
The additional ability in this phase is called Explosive Cinders. It is a debuff put on a random person (only 1 person), which explodes to deal about 50k damage to others within 10 yards and knocks them back, effectively killing them. If you get this debuff, run into the lava after it has about 4 seconds on the duration, and let it explode. SWIM through the lava, do NOT jump. It'll do less damage if you swim. Jumping adds an extra stack each time. Once the ability blows up, get back on the platform and hope that you don't die. One of these abilities will go out right as the phase begins. This one is always the trickiest, and the person who gets it should stay in the lava until it goes off, rather than swimming up to the pillar first. Big heals are required on this target as he/she will have around 10 stacks of the lava. Make sure that this person is far away from the pillar too, not right up against it.
I recommend two interrupters per pillar. One with a 10 second or less CD interrupt, and one backup. All melee classes work perfectly as your "less than 10 second" interrupter, and paladin healers can also do this. Back-up mages are great, as their CD is long, but able to be cast at range. We tended to put our tanks as interrupters (DK and paladin) and have our melee be the backups. Arcane Torrent works as well, bloodelves. We took each of our adds down to 600k health. Then we nuked them. Make sure they're about even before nuking them to death. The first death starts phase 3.

Phase 3
If you're lucky, the adds from phase 1 will be in a neat pile and the offtank will have no trouble picking them up. This phase is 1 MILLION% on the offtank who's kiting the adds. I would run them to the outside ring and run them in a large circle around the place. Have a healer who is designated to keep him up. This healer needs to be running around with him. Always be moving, and don't walk backwards. If the adds get touched by fire even once you will be ripped to shreds by them before they can die again. Make sure that the healer uses a CD on the offtank when each electrocute comes out, as it hurts pretty badly. We often had electrocutes kill the offtank resulting in a wipe. If the offtank dies, don't bother brezzing him. It's a wipe. The adds will first kill the offtank healer in 1 shot, then run to the raid, which is through fire, and be unstoppable.
Dominion is still in effect in phase 3. Use it to your advantage, DPS increase! Arcane mages have 1 million crits with the buff, so it really boosts DPS. Once Nef gets below 10 million, lust it up and blow him up. At that point, he has 2 electrocutes left in him and he will die before the last one goes off (probably). So just kill kill.
Pick your add tank wisely. If the add tank is bad, it will be a phase 3 wipe EVERY TIME. The add damage is unforgiving if they are allowed to stay alive.

Last Notes: Phase 2 is easily the hardest one to heal, and the person who first gets the Explosive Cinders on them often dies. If cinders get on your interrupter, make sure that the backup is ready to do the interrupt. Letting one tick get off on one interrupt will most likely cause a few deaths, and a raid wipe if it's at the beginning of the phase. It just puts healers so far behind. Don't miss your interrupts. Make sure people with Cinders move OUT, and kite adds well in phase 3. That's what really makes this fight possible.
